Obtaining a device through Marine Medical

All equipment issued requires a signed Letter of Medical Necessity (LMN) from a provider (MD, PAC, DO, DC, ND, ARNP).  This letter states that the device is recommended and necessary secondary to your medical condition.  Upon receiving a device, your therapist or customer service representative from Marine Medical will have a Patient Information Form (PIF) or an Assignment of Benefits (AOB) for you to sign.
